Annual Wellness Nurse LPN

Schedule: Monday - Friday 7:45am - 4:15pm. No weekends/holidays General Summary Works in collaboration with the primary care providers and all members of the health care team. Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of the guided processes to complete the AWV. Assesses healthcare needs that require additional follow up by primary care provider and assists patients in scheduling appropriate follow-up. This position requires expertise in the nursing process, using critical thinking skills, plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations as a member of the clinical team. Must be an innovative thinker and open to changing the way we deliver care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/staff education. Provides safe, therapeutic, efficient quality care and services to all patients served. Functions as a member of the health care team at licensed nurse level to assist in the planning and implementation of patient care, based upon sound nursing judgment, clinical preparation, knowledge, skills, and experience.

Case Manager Registered Nurse - Emergency Department

Every-third weekend will be in the ED for 12 hours General Summary Performs a variety of duties and applies utilization and case management techniques to determine the most efficient use of resources to facilitate a patient's progression of care and the provision of appropriate, cost effective and quality health care. Works in a team model with social work in assigned patient care areas to address patient discharge needs. Provides leadership in the integration of utilization and case management principles.

Our patients are babies needing advanced care for prematurity, whole body cooling, surgery, hypoglycemia, sepsis, and more. Our NICU is a regional transport center for ill newborns. General Summary Provides supervision of the daily operations and work activities of professional nursing and ancillary services staff on assigned unit. Able to support ongoing operations of department/unit independently as needed. Provides direct patient care as required.

Certified Nursing Assistant - Critical Care

Critical Care Unit Part Time (24 Hours Biweekly) 1 12-hour Day shift every week 0545-1815 WellSpan Waynesboro ICU is a 10-bed unit which provides care to critically ill adult and geriatric patients. As the only intensive care unit in WH, we provide care to a multitude of clinically complex patient populations. We specialize in caring for those who require ventilator or other acute respiratory support, pressors and inotrope therapy, TNK administration for stroke or pulmonary emboli, and hemodynamic monitoring. General Summary Provides clinical support services to patient care staff by performing a variety of duties related to patient care. Takes patient vital signs, assists patients with activities of daily living (e.g., bathing, feeding, toileting, etc.), inventories and replenishes supplies and equipment, and performs special assigned duties such as performing EKGs and Point-of-Care testing.
