Job Responsibilities

Providing post-acute LPN nursing care to residents with varying medical needs
Tracking the vital signs, physical condition, and mental acuity of residents
Completing intakes of residents, including the documentation of medical histories and health concerns
Assisting with medications, including proper administration, verification, and monitoring

This position is responsible for: The LPN acts as a liaison between the patient and physician/provider. The LPN assists physicians, other nurses, and additional medical care team members in the delivery of care to pediatric patients. Performs complete and accurate patient assessment or reassessment.
