Job Summary: Coordinates activities of personnel on assigned shift(s) to maintain continuity of care, patient flow and throughput accountability. Arranges for staffing levels according to acuities and census. Assesses, plans, implements and evaluates nursing and hospital standards and unit policies and procedures for the purpose of monitoring and evaluating patient functional status.

Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License.

Essential Functions and Responsibilities: Uses the nursing process and physician orders to develop individualized patient care plans. Provides care/interventions based upon the individual care plan; continually assesses the effects of interventions, medication and treatments. Participates in discharge planning. Assists in guiding/directing nursing staff in utilizing these processes. Documents steps in the nursing process in the medical record utilizing appropriate medical terminology and hospital-approved abbreviations. Reports observations and findings regarding patients to the appropriate person and records those observations accurately and concisely. Complies with all health system, hospital and unit safety guidelines. Coordinates maintenance of safe and satisfactory environmental conditions for staff and patients through continuous monitoring of unit for compliance with safety and infection control programs, policies, and procedures. Verbalizes understanding of patient safety goals as applicable to unit. Responds effectively to patient emergencies. Applies teaching/learning principles in the promotion of patient health education based upon identified goals. Provides patient/family education throughout hospital stay and at time of discharge to address learning needs associated with tests, procedures, hospital routine, medications post-discharge care, etc. Assists in guiding/directing nursing staff in utilizing these processes. Investigates patient and staff complaints; investigates patient care incidents and reports to nurse manager; functions as a patient and family advocate at all times. Completes variance reports according to established guidelines to document unusual occurrences. Meets system, hospital and unit-specific competencies annually. Supports system/hospital/unit goals by attending staff meetings, in-services and educational offerings. Practices self-direction by assuming responsibility and accountability for professional growth. Participates in: economical utilization of supplies and ensures that equipment is maintained in a clean/safe manner; orientation of new personnel and supports assimilation of new team members; performance improvement process by assisting in monitoring quality, appropriateness, patient care; projects/processes designed to improve patient outcomes. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This includes providing nursing assessments, planning, interventions, evaluations, health related education, case management, and execution of the medical regime including administering medications and treatments prescribed by any person authorized to prescribe by state law. Typical Functions The functions performed in this job family will vary by level, unit and organization, but may include the following: Assesses health status of individuals, families, and communities; develops plans and implements appropriate nursing interventions. Evaluates and determines health resources necessary to meet individual, family and community health needs. Delivers professional nursing care in an assigned unit, clinic, home, or other setting. Educates individuals, families, communities, and members of the health care team about the principles of disease prevention and health promotion. Provides supervision to other professional or paraprofessional personnel; collaborates with other professionals in the management of health care. Delegates tasks as may safely be performed by others, consistent with educational preparation and that do not conflict with the provisions of the Oklahoma Nursing Practice Act. Assures quality health care through use of various measures such as record review, peer review, direct observation, and assessment of individual, family, and community for the desired outcome. Performs specialized nursing functions as educationally prepared. Performs other related duties as assigned consistent with the responsibilities of the position and agency operations. Level Descriptor Levels are distinguished based on the complexity of assigned duties, the level of expertise required for completion of work assignments, and the responsibility assigned for the supervision of others. This is the senior level of this job family where employees are assigned more advanced responsibility in an assigned work unit or in an assigned geographic area and may be assigned responsibility for direct supervision. Employees at this level will have complex assignments and a substantial degree of responsibility and independence in planning, organizing, and completing work. Employees at this level develop and present educational programs relevant to nursing practices and standards or care and follow-up with evaluation of learning. This position may include completing special assignments such as quality improvement, mentoring, and surveillance of potential patient care incidents of quality indicators for individuals who may be at risk for rapid status change. Responsibilities may include scheduling work assignments, evaluating health care and performance of assigned staff, providing training, and otherwise insuring quality. Administrative responsibilities may include approving leave and initiating disciplinary actions. Positions which are assigned independent responsibility for coordinating health services or providing nursing care in an assigned county or geographic area or as the only nurse in that location may also be included in this level. Employees at this level deal with complex health issues and may act for nurse managers in their absence. Education and Experience Education and Experience requirements at this level consist of possession of a valid Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing and two years of professional nursing experience in a field of nursing appropriate to the health care setting; or Registered Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) and two years of professional nursing experience in a field of nursing appropriate to the health care setting. Knowledge, Skills, Abilities and Competencies Knowledge, Skills and Abilities required at this level include knowledge of professional nursing theory, practices, and techniques; of health promotion and disease prevention; of nursing management of available health care resources; of nutrition and diet therapy; of the functions of various clinical departments; of available health programs and resources; of federal and state rules and regulations concerning health services; of accreditation, certification and licensure standards; of preventive health practices; of principles and methods of teaching; of nursing standards; and of nursing policies and procedures. Ability is required to assist in the development, organization, and implementation of an effective nursing care plan; to provide education and training; to assess condition and needs of patients, families and communities appropriate to the care setting; to take appropriate intervention; to counsel and teach patients and families; to provide direction or guidance to non-licensed personnel in assigned area of responsibility to ensure quality care; to develop, plan, and implement an effective nursing care program; and to provide education and training. Candidates must possess sufficient proficiency in the English language to perform the essential functions of the position. Special Requirements Upon appointment, incumbents designated by the Oklahoma Department of Mental Health and Substance Abuse Services shall complete certification in one of the following required Departmental sponsored training in case management: Certified Behavioral Health Case Manager – Adult; Certified Behavioral Health Case Manager – Child, Adolescent and Family; or Certified Behavioral Health Case Manager – Dual Certification. Upon a conditional offer of employment, Department of Mental Health and Substance Abuse Services applicants must submit to a physical examination and shall meet the physical examination requirements of occasionally lifting/carrying 50 pounds and able to push and pull a maximum force of 25 pounds. Applicants with the Department of Corrections must be willing and able to work any assigned shift or day of the week, and additional shifts as required.
