Responsibilities

  • Visit patients in their homes for one-on-one visits
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients 
  • Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
  • Administer medication as prescribed by the patient’s physician  
  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

  • Graduate of an accredited Diploma, Associate, or bachelor’s School of Nursing, or vocational nursing
  • Current state license as an RN
  • Proof of current CPR certification 
  • Compassionate, friendly demeanor 
  • Excellent communication skills
  • Current driver’s license and reliable transportation 
  • Bilingual preferrred

Clinical Documentation Improvement Specialist RN

Position Summary Facilitates improvement of clinical documentation. Obtains, through the query process, appropriate physician documentation that identifies clinical conditions or procedures and supports the severity of illness, risk of mortality, complexity of care, and appropriate reimbursement for the level of services rendered. Supports timely, accurate, and complete documentation of clinical information used for measuring and reporting physician and hospital outcomes. Educates all members of the patient care team on an ongoing basis. Qualifications Inpatient experience or other applicable clinical experience with excellent written and oral communication skills as well as critical thinking skills. Clinical knowledge (anatomy and physiology, pathophysiology, and pharmacology) and working knowledge of or ability to learn inpatient admission criteria. Knowledge of or ability to learn coding concepts and guidelines and clinical terminology, ability to read and analyze all information in a patient’s health record.
