Our team consists of experienced, energetic, dedicated and compassionate RNs, LPNs, and STNAs.

 Job Qualifications:

  • Current licensure as a Registered Nurse in the state in which practicing.
  • Graduate from an accredited School of Nursing.
  • Sufficient verbal and written communication skills to perform the tasks required.
  • Ability to apply basic principles of nursing care based on standards of nursing practices.
  • Maintain current certification in cardio-pulmonary resuscitation.
  • Minimum of one (1) year of experience in nursing supervision in long term care which includes direct care of the aged or chronically ill.

Job Requirements & Duties:

  • Ensures 24-hour provision of nursing services within federal, state and company requirements based on acuity of residents.
  • Maintains standards of nursing services within the expectation of the Nurse Practice Act.
  • Acts on behalf of Director of Nursing Services in her absence.
  • Ensures exchange of essential information necessary for the provision of quality resident services.
  • Ensures that all documentation necessary for quality resident care is maintained according to federal, state and company policy,
  • Reviews daily assignments with Director of Nursing Services, RN Supervisors and Charge Nurses.
  • Monitors nursing personnel performance.
  • Makes daily rounds, identifies problems, initiates corrective actions and monitors follow up.
  • Supports the overall goals of the company and facility.
  • Provides counseling and disciplinary action to all levels of nursing personnel as situation warrants. Makes recommendations to Director of Nursing Services if further disciplinary action is warranted.
  • Coordinates Nursing Services with other resident services.
  • Recommends number and level of nursing staff to be employed to Director of Nursing Services.
  • Evaluates and identifies nursing personnel for development and/or educational needs.
  • Ensures that only those residents, for whom services can be provided, are admitted to the facility.
  • Maintains Nursing Policy and Procedures Manuals.
  • Develops Nursing Policy and Procedure as needed and in compliance with Corporate Standards.
  • Develops and maintains written task guidelines for all nursing personnel.
  • Attends and participates in meetings and in-services as required.
  • Assists in development and maintenance of a budget for Nursing Services as defined by facility's budget.
  • Ensures that the nursing functions and training of staff are met.
  • Other responsibilities as delegated by immediate supervisor.
  • Establishes department work goals and coordinates delivery of care through supervision of nursing personnel.
  • Supervises all nursing personnel.
  • Evaluates nursing personnel performance as designated by Director of Nursing Services.
  • Coordinates care plan meetings to assess the total resident needs, including spiritual, emotional, psychosocial functional factors, encouragement of self-respect and esteem, and guidance toward implementation of restorative nursing measures.
  • Makes decisions in emergencies and is accountable for decision-making.
  • Monitors the inventory and ordering of supplies to meet resident needs.
  • Conducts systems reviews and staff meetings with licensed staff to provide input and feedback on progress of assigned system responsibilities.
  • Responsible for nursing personnel adherence to company and healthcare industry standards in the area of consumer service as designated by Director of Nursing Services.
  • Ensures that all consumer resident rights are protected.

The position also collaborates, in a collegial manner, with physicians and other health care team members in meeting the patient’s needs. This position may delegate certain aspects of care to those under their direct supervision, within the scope of nursing practice. Job Responsibilities Performs initial and ongoing assessment of patient and family. Completes initial assessment tool. Documents ongoing assessment per unit/TriHealth guidelines. Communicates assessment findings to other health care providers as appropriate. Includes health counseling and health teaching needs in assessment. Plans care for patient and family based on assessment, standards of care, and optimal specific outcomes. Initiates and individualizes appropriate patient care guidelines/plan of care or clinical pathways. Updates current plan of care as needed based on patient/family input and healthcare needs. Develops both short- and long-term goals with patient/family and healthcare team including discharge planning. Communicates plan of care to others. Provides a safe, therapeutic environment, maintains patient’s autonomy, dignity, and rights, and is sensitive to patient diversity. Seeks resources to help formulate ethical decisions. Balances priorities of the patient's needs and those of the unit/facility. Recognizes emergency situations and takes appropriate action. Completes patient assignment including documentation within scheduled timeframe. Bases interventions on clinical data and desired outcomes and documents accordingly. Trains/educates other staff and acts an expert resource in specialty area through abilities in existing and newer knowledge and skills. Effectively communicates and understands/executes physician orders. Evaluates the plan of care for patient based on optimal specific patient outcomes. Documents the patient/family response to care including teaching. Collaborates with the patient/family and with other members of the health care team, including physicians, to revise plan of care as needed. Supervises the care that was delegated to other health care team members. Performs technical skills according to policy and procedure and accepted standards within their area of practice. Safely administers medications/treatments and monitors their effects. Uses all equipment in a safe, appropriate manner. Demonstrates organizations responsibilities: Identifies areas for self-improvement, functions in relief charge / resource role as requested, completes assignments within scheduled timeframes, cooperates with instructor to facilitate effective learning experiences for students, maintains current knowledge in area of practice, demonstrates knowledge of organizational and department changes.
