Job Summary:

Responsibilities:

Provide compassionate, resident-centered nursing care in accordance with individual care plans
Administer medications and treatments as ordered and according to facility policies
Monitor residents and promptly report changes in condition to the RN Supervisor or appropriate clinical leadership
Perform nursing assessments and document resident care accurately and timely
Assist with activities of daily living and promote resident comfort, dignity, and independence
Collaborate with CNAs, RNs, physicians, therapists, and other members of the interdisciplinary team
Follow infection prevention and control procedures and maintain a safe environment
Participate in care planning and communicate relevant resident information to the healthcare team
Maintain accurate clinical documentation and medication records
Respond appropriately to resident and family concerns
Follow all facility policies, procedures, and applicable state and federal regulations
Promote a positive and respectful environment for residents, families, and coworkers

Qualifications:

Current and valid New Jersey LPN license required
Previous experience in a skilled nursing or long-term care setting preferred, but not required
New graduates with an active NJ LPN license encouraged to apply

Ability to work effectively as part of an interdisciplinary healthcare team
