About the Role:

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our evening shift team. As an LPN, you will provide direct patient care, administer medications, and develop and implement individualized care plans in collaboration with our interdisciplinary team. Your expertise and passion for delivering exceptional care will enable our patients to achieve optimal wellness and independence.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital signs monitoring

  • Develop and implement individualized care plans in collaboration with the healthcare team

  • Maintain accurate and up-to-date records of patient care and treatment

  • Participate in interdisciplinary team meetings to discuss patient progress and develop care strategies

  • Work closely with our rehabilitation and subacute care teams to ensure seamless transitions and continuity of care

  • Demonstrate respect, empathy, and understanding in interactions with patients, families, and colleagues
