Home Care Aide - NYS Home Health - Per Diem

$17 - $23.85 / hour
Job Description

Position Summary: Provides personal care and assists with activities of daily living and homemaking to Long Term Home Health Care patients in their homes following a care plan developed by a Primary Case managing RN.. Is responsible for following the aide care plan, reporting issues related to providing care and patient symptoms as directed in the plan.

Education, License & Cert: High School Diploma or GED Required Completion of a NYS DOH approved certification program and is in good standing

Experience: Minimum one year of relevant experience. Experience with public contact is preferred.

Essential Functions: 1) Updates Knowledge and documentation per agency standards

  • Understands and adheres to GCMC’s compliance standards as they appear in GCMC’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
  • Attends in‐service programs designed to update knowledge and skill related to patient care and to keep abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they change or are modified relevant to job
  • Reads aide care plan at the beginning of each home visit and performs all those tasks listed on care plan and follows policies of LTHHCP in the delivery of care. If care plan tasks can’t be accomplished or the patient requests other tasks, the aide is to call the LTHHCP office for authorization of changes
  • Accurately and completely prepares report forms provided by the agency and requests that the patient sign the report sheet after each visit per agency policy. The aide must sign the report sheet indicating which tasks were provided
  • Observes infection control protocols per agency policy utilizing personal protective equipment provided by the agency
  • Familiarize self and seeks‐out instructions as needed in relation to all patient care tasks and equipment. Notifies LTHHCP office and Nurse if unfamiliar with a patient care task and or equipment found in patient’s home. Request an orientation to new or unfamiliar cases and tasks
  • Provides Personal Care as directed
  • Provides personal care (help with grooming, bathing, toileting, eating and dressing ‐ includes routine care or nails and hair and oral hygiene) that keeps patients clean and well groomed and prevents skin breakdown as directed on the plan
  • Provides nutritional support through preparation of meals following diet plan prescribed in care plan and following preferences expressed by patients
  • Provides for patient’s comfort needs ‐ back rubs, beds and pillows adjusted, etc
  • Reports any care issues and concerns to the LTHHCP office promptly
  • Accompanies patients to Doctor appointments when directed and reports visit to the Primary Nurse when visit is complete
  • Assists in the management of environment of care
  • Respects the patient’s home and environment and establishes a professional but friendly and supportive relationship with the patient
  • Does laundry for patients with special care given to preference of patients and content of laundry
  • Maintains clean environment of patient care area or home. Completes all items on the care plan (i.e., changing bed linens, moping, sweeping, dusting, cleaning of patient care equipment, dishes, bathrooms etc.)
  • Develops shopping lists in conjunction with patient and shops for patient. Encourages the patient to follow the diet plan prescribed in the care plan
  • Completes requirements per agency policy
  • Utilizes receipt forms when handling money for patients following LTHHCP policy.
  • Picks up work schedule each week per agency policy and accurately keeps record of actual arrival and leaving times for each case on forms provided by the agency. Turns these in at the end of each work week
  • Provides accurate mileage utilizing forms provided by the agency and turns them in every two weeks/end of pay period
  • Performs other duties and tasks as requested and directed by the agency that assists colleagues and patients of the LTHHCP
  • Performs other duties as requested and observed by manager or supervisor


Pay Rate ranges from $17.00 - $23.85 per hour.
