Your Role as an MDS Care Plan Nurse (LPN/RN):

  • Complete timely and accurate MDS assessments in accordance with federal and state regulations

  • Collaborate with interdisciplinary team members to develop individualized resident care plans

  • Monitor care plan implementation and ensure coordination of services
