About the Role:

We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Care team. This role includes both field nursing visits and admissions work, providing quality care and support to patients in the comfort of their own homes.

Responsibilities:

  • Conduct patient assessments and complete admission documentation.
  • Develop, implement, and evaluate individualized care plans.
  • Provide skilled nursing care, including medication management, wound care, and patient education.
  • Collaborate with physicians, case managers, and home health aides to ensure coordinated patient care.
  • Maintain accurate and timely clinical documentation in compliance with agency and state regulations.

Qualifications:

  • Active New York State RN license (required).
  • Minimum of 1 year of home care or clinical nursing experience preferred.
  • Strong communication, organizational, and clinical assessment skills.
  • Valid driver’s license and reliable transportation.
