Hospice Registered Nurse Case Manager (RN)

Now Hiring – RN Case Manager 

Starting Salary:  90,000/Year 

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

  1. Completing an initial assessment of patient and family to determine hospice care needs
  2. Regularly re-evaluates the patients nursing needs
  3. Use assessment data to determine care plan needs
  4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
  5. Counsels the patient and family in meeting end of life needs
  6. Initiates appropriate preventative and palliative nursing procedures
  7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
  8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

 

Professional Experience:

  • Must have a State Certified RN license
  • 2 years' experience as a professional nurse, hospice experience preferred
  • Must have a valid Driver's license and reliable transportation
  • Must be willing to travel
  • Strong verbal and written communication skills.

This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own on a day-to-day basis with activities which include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Responsibilities Provides professional nursing care to patients within unit. Diagnose and treat response to actual or potential health problems by means of nursing techniques and methods such as case finding, health teaching, health counseling and the provision of care supportive to and restorative of health and well-being. Maintain safe levels of care while adhering to a medical regimen. Collaborates with the physician and other health care providers to assure that a goal oriented care plan is consistent with medical regimen and hospital procedures. Performs other related duties as instructed. Qualifications BSN with three (3) years’ experience preferred. Current NYS license to practice as a Registered Professional Nurse. BLS Required. Bilingual Preferred: English/Spanish.
