Duties Description The Registered Nurse 2 Community oversees the medical care for individuals living in Family Care (FC) homes. They are required to visit each home as frequently as health care needs warrant but in no case less than once every 90 days with the individual present. The candidate will communicate routinely with Family Care Provider to provide support as needed. They will participate in the development of recommendations related to home modifications and adaptive equipment as appropriate. They will assess Family Care Provider skills and abilities relative to addressing the needs of individuals in their home. Ensures Medical Record and Ready to Go Binder in FC home contains all required documents, is orderly and current for each individual in the home. Candidate will coordinate appointments for individuals, ensure follow up and documentation from appointments is obtained. During a hospitalization the RN2 Community is required to follow up with hospital personnel regarding the individual and provide updates on a consistent basis.
