In this role, you will:

  • Conduct a comprehensive assessment of individuals in their homes.
  • Develop individualized care plans based on assessed needs.
  • Provide in-service training to Direct Support Professionals (DSPs) on individual care plans and best practices.
  • Conduct monthly check-ins with individuals to monitor their well-being and ensure quality of care.
  • Maintain accurate and up-to-date client records.
