RN Clinical Manager for Pediatric Home Health

We are seeking a dedicated and experienced Clinical Manager (RN) to oversee the quality of care for our pediatric home health clients. This is an in-office position responsible for case managing our pediatric clients who are served in the home, ensuring our nurses and home health aides deliver safe, high-quality care that improves patient outcomes and maintains full regulatory compliance.

Position Summary

The Clinical Manager serves as the clinical hub for our pediatric home health program. From our office, you will coordinate and oversee the care delivered in our clients' homes, supervise field nursing staff and aides, monitor visit compliance, and drive quality improvement across the department.

Key Responsibilities

• Manage and coordinate the plan of care for an assigned caseload of pediatric home health clients

• Supervise and support field nurses and home health aides, ensuring care is delivered according to physician orders and individualized care plans

• Oversee visit compliance , ensuring nurses complete scheduled visits, assessments, and supervisory visits on time and per regulatory requirements

• Review documentation for accuracy, completeness, and compliance with state, federal, and payer standards

• Monitor and drive clinical outcomes, identifying opportunities to improve quality of care

• Conduct and oversee assessments, reassessments, and care plan updates

• Serve as a clinical resource for field staff, families, physicians, and the interdisciplinary team

• Participate in quality assurance and performance improvement (QAPI) initiatives

• Coordinate with scheduling and intake to ensure appropriate staffing for each case

Qualifications

• Active and unrestricted RN license

• Minimum 1 year of experience in a supervisory or leadership role

• Pediatric and/or home health experience strongly preferred

• Strong knowledge of home health regulations, documentation standards, and compliance requirements

• Excellent organizational, communication, and clinical assessment skills

• Ability to manage multiple cases and priorities in a fast-paced office environment
