Caring Angles Home Care is seeking a dynamic Geriatric RN Case Manager with Oasis experience for our Brooklyn office. As Case Manager, you will play an integral role in the day-to-day functions of the agency. This position offers a competitive salary and benefit package. Case Manager Responsibilities: The Case Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Case Manager is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel. Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs.
