Job Description

Basic Purpose

Positions in this job family are assigned responsibilities for providing direct and indirect non-professional nursing care or outreach services in a clinic, hospital, institution or home setting under nursing or medical supervision. This includes performing duties such as taking vital signs, obtaining lab specimens, charting, escorting patients, assisting with activities of daily living, and ordering and receiving supplies.

Typical Functions

  • Performs direct nursing tasks including taking vital signs, assisting with activities of daily living and hygiene needs, and obtaining certain lab specimens.

  • Charts observations and activities, reporting pertinent changes in the patient’s condition.

  • Performs delegated or other specialized functions as educationally prepared.

  • Escorts patients to and from various destinations.

  • Assists in maintaining and providing a clean, safe environment.

  • Orders, receives and stores supplies and performs basic clerical functions.

  • Participates in follow-up activities and tracking clients, schedules appointments and performs other related tasks.

  • Provides case finding and recruitment of at risk persons for comprehensive health services.

Level Descriptor

This is the leadership level of this job family where employees are assigned responsibility for providing guidance, training, and assignment of work activities to other patient care assistant staff, and perform more advanced duties.

Education and Experience

Education and Experience required at this level consists of two years of experience in providing patient care.

Knowledge, Skills, Abilities and Competencies

Knowledge, Skills and Abilities required at this level include knowledge of the English language in order to communicate effectively, both orally and in writing; of public health services available to the community; of record keeping; of the techniques of interviewing, of basic nursing practices and techniques; of basic methods of sanitation and personal hygiene; of medical terminology; and of planning and organizing work assignments. Ability is required to record observations and activities, to follow oral and written directions; to establish and maintain effective working relationships with others; to apply basic nursing techniques; to monitor and direct the work of others; and to judge a situation accurately and adopt an effective course of action.

Special Requirements

The Department of Veterans Affairs requires current certification in good standing by the Oklahoma State Department of Health as a (CNA) Certified Nursing Aide for Level I and II. Level III requires certification as a (CMA-A) Certified Medication Aide Advanced by the Oklahoma State Department of Health. Be on feet most of 8 hours with two 15 minute breaks and one 30 minute break; walk, squat, bend, and kneel; access stairs; use hands and fingers to grip and hold; use arms for reaching and extending in a full range of motion in providing personal care, grooming, feeding tasks and/or administering medication and treatments to patients; physically turn, position, ambulate, and transport patients with mechanical lift (routine care) independently or with assistance; lift and carry up to 25 pounds without assistance; and push or pull (maneuver) wheelchairs, geri chairs, carts, etc. weighing up to 500 pounds or 25 lbs. of force as measured by a Chatillon force gauge or similar device.  In emergency situations must transfer patients with 2, 3, or 4 persons lift technique on level surface or stairs.

Description Job Summary This position is responsible for both the rendering of basic supervised nursing care to patients in support of the Physicians as well as accurate and efficient data/demographic information entry into software programs utilized by office and hospital. This position maintains accurate and up-to-date filing of patient data within the clinic chart to allow efficient review on-demand. During patient office appointments, this position serves to maintain quality patient care. Responsible for pleasant and effective communication with the public as well as assisting with timely and efficient patient appointments. Responsible for ensuring and assisting with efficient patient flow throughout the clinic day. Duties and Responsibilities · Performs administrative duties which include, but is not limited to: Scheduling patients’ appointments, requesting and sending medical records as needed, phone calls to confirm existing appointments; · Answers phone –verifying current demographic information at each visit. Responsible for taking messages or refers incoming telephone calls to the appropriate administrative or clinical staff member for processing. · Assists in scanning duties: Requests medical records for new clients as needed. Readies electronic charts for appointments as needed. · Assists in keeping clinical office inventory and notifies supervisor when supplies need to be ordered. Maintains inventory of necessary office supplies. · Updates and maintains medical records in preparation for patient appointment and is responsible for routing incoming patient data to Physician for evaluation/review, and to the Nurse for computerized update of clinical problem list. · Makes charts for new clients as needed. Make photocopies of records for consultations or referrals as directed by supervisor. · May order refills for prescriptions with the Physician’s permission and supervision. · Places patient in the examination rooms and prepares patient for appointment. Takes and records routine elements of systemic reviews: elements of patient’s history, elements of present illness. Takes vital signs of the patient. · Maintains inventory of necessary medical/examination supplies needed for each of the examination rooms. Restocking of examination rooms (per posted inventory list) will be done daily, more frequently as needed. · Straightens up examination rooms after patient examinations.
