Nurse Practitioner

As the Nurse Practitioner you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.

You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.

As a Nurse Practitioner, you will provide types of care, including but not limited to:

  • Be comfortable and proficient with documentation and CPT coding
  • Evaluation of new-admission visits
  • Coordination of discharge visits
  • Monthly stability visits for long term residents
  • Evaluations for patients exhibiting changes in condition
  • Advanced Care Planning

Other Duties & Responsibilities

  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with attending physicians and/or specialists regarding patient care
  • Be comfortable and proficient with documentation and CPT coding

Requirements

Licensure / Certification / Registration:

  • Advanced Practice Licensure (APN) with an applicable state license
  • Must have an active license and DEA #
  • Previous experience in nursing or other medical fields
  • Minimum one-year experience in a Skilled Nursing setting (preferred)
  • Ability to build rapport with patients and their loved ones
  • Ability to thrive in a fast-paced environment
  • Excellent written and verbal communication skills
  • Strong leadership experience with emphasis on teamwork in a healthcare setting

Education
