Responsibilities:

Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement care plans in collaboration with the healthcare team
Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care
Participate in Shift Reports and handovers to maintain continuity of care
Collaborate with Nursing Assistants and other caregivers to ensure a safe and supportive environment
Monitor and report any changes in patient condition or concerns to the Nursing Supervisor
Maintain accurate and up-to-date patient records and reports

Requirements:

Current LPN licensure in the State of Arkansas
Minimum 1 year of experience in a long-term care setting
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Basic computer skills and experience with electronic medical records
