HudsonView Center is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team in Hudson County, NJ. This is an impactful role centered on providing high-quality, patient-centered care within post-acute and long-term care settings. Collaborate with a supportive interdisciplinary team to drive excellent patient outcomes.

Role Focus & Responsibilities

The primary focus is managing complex medical conditions to reduce hospital readmissions and ensure seamless patient transitions.

  • Clinical Care: Conduct comprehensive patient assessments, diagnose conditions, and develop and manage complex care plans.
  • Documentation: Accurately document all patient encounters and treatments using Electronic Health Records (EHR).
  • Collaboration: Work seamlessly with physicians, nursing staff, and other healthcare professionals.
  • Communication: Serve as a key communicator with patients’ families and support systems.
  • Participation: Attend facility meetings and actively contribute to care planning discussions.
  • Standards: Maintain the highest level of professionalism and uphold HudsonView Center's commitment to clinical excellence.

Qualifications

Required

  • Education: Master’s or Doctorate degree in Nursing from an accredited program.
  • Licensure: Active New Jersey APN and RN Licenses.
  • Certifications: Active DEA, CDS, and BLS certifications.
  • Experience: Minimum 2 years of Nurse Practitioner experience.
  • Expertise: Proven experience managing elderly patients and complex medical conditions.
  • Tech: Proficiency in electronic health records (EHR).

Preferred

  • Experience in post-acute care environments.
  • Strong procedural skills, including: IV insertion, G-tube replacements, wound care, and suture/staple removal.
