Responsibilities:

  • Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
  • Diagnose and develop individualized treatment plans based on the patient's mental health needs.
  • Provide medication management and adjust treatment plans as necessary.
  • Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
  • Offer counseling and psychotherapy services to patients and their families.
  • Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
  • Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
  • Participate in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

  • Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
  • Ability to work independently and effectively manage a caseload of patients.
  • Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
  • Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
  • Proficient in electronic health record (EHR) systems and basic computer skills.
  • Compassionate, patient-centered approach to care.

Educates patients on treatment plans, health maintenance, and development. Orders and interprets diagnostic tests to assess clinical conditions. Collaborates with physicians and healthcare teams for comprehensive care. Collaborates with Medical director and client stakeholders on Health Promotion programs, surveillance needs, ergonomics, process development, and other related safety/injury prevention opportunities. Refers patients to specialists or physicians as needed. Maintains patient health records ensure complete, accurate and up-to-date documentation and records. Educates and coaches nursing staff on best practices. Performs other duties as assigned. What You’ll Bring Maintains state RN licenses/certificates to practice. Maintains NP licenses/certificates to practice. Maintains State Prescriptive Authority (if applicable) . Federal DEA license (matching each state of practice as applicable) . State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration). Current national certification (ANCC or AANP required). Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required). Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services. Certification in occupational health (COHN/COHN-S/COHN-S/CM) preferred. At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting.
