Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. We are a leading healthcare organization providing comprehensive mental health services to patients in nursing homes across Nassau County, NY. Responsibilities:

Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
Diagnose and develop individualized treatment plans based on the patient's mental health needs.
Provide medication management and adjust treatment plans as necessary.
Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
Offer counseling and psychotherapy services to patients and their families.
Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
Participate in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
Ability to work independently and effectively manage a caseload of patients.
Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
Proficient in electronic health record (EHR) systems and basic computer skills.

How you make a difference The Psychiatric Nurse Practitioner provides a full range of psychiatric services to incarcerated patients in accordance with state licensure, collaborative practice requirements, and established policies and procedures. This role delivers direct psychiatric care, clinical consultation, and follow-up services to support patient treatment and continuity of care. The position works closely with mental health, medical, and administrative teams to ensure consistent and compliant psychiatric services. The Psychiatric Nurse Practitioner also seeks clinical consultation and supervision as required by collaborative practice protocols. Key Responsibilities Provide psychiatric evaluations, treatment, follow-up care, and medication management within professional and regulatory standards. Monitor effectiveness and side effects of psychotropic medications and provide ongoing patient education and informed consent. Collaborate with mental health, medical, and administrative teams to ensure coordinated and consistent psychiatric services.
