Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. Responsibilities:

Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
Diagnose and develop individualized treatment plans based on the patient's mental health needs.
Provide medication management and adjust treatment plans as necessary.
Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
Offer counseling and psychotherapy services to patients and their families.
Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
Participate in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
Ability to work independently and effectively manage a caseload of patients.
Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
Proficient in electronic health record (EHR) systems and basic computer skills.

How you make a difference The Psychiatric Nurse Practitioner provides a full range of psychiatric services to incarcerated patients in accordance with state scope of practice, collaborative protocols, and established policies. This role delivers direct psychiatric care while also providing clinical consultation to mental health and healthcare staff. The position works within state guidelines to support consistent, high-quality psychiatric services across the facility. Clinical consultation and supervision are sought as required under applicable collaborative practice requirements. Key Responsibilities Provide psychiatric evaluations, treatment, and follow-up care while exercising independent medical judgment. Monitor and manage psychotropic medications, including effectiveness, side effects, and required follow-up.
