Overview Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes. Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement. Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner. Qualifications Education: Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing. Experience: Minimum one (1) year clinical experience preferred. Position Summary: Assist physician with duties necessary to support the needs of the practice working within and to the maximum scope of license Education, License & Cert: LPN ‐ license based on corresponding position location Experience: One‐year experience in healthcare field preferred but not required Essential Functions: 1. Directs patient to exam room and prepares patient for exam or treatment by identifying height, weight, respirations, blood pressure, temperature, additional testing, and records same on chart.
