Description

The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center.   This individual interfaces directly with Mount Sinai Health System Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice


Responsibilities

Responsible For
Development, implementation, coordination and evaluation of clinical programs and services related to core measures.
Principal Duties and Responsibilities
Clinical Leadership
1.    In conjunction with Nursing, Medical and Quality Leadership, coordinates core measures and clinical intervention compliance activities.
2.    Assists in development and design of action plans to support compliance with measures indicators and provides support and educational resources to departments and services relative to core measures quality assessment and improvement processes.
3.    Assists in establishing and implementing short and long term goals, objectives, policies and operating procedures for core measures programs.
4.    Provides coordination and leadership in the execution of day to day program activities, as appropriate to program objectives and areas of expertise.
5.    Oversees the collection, compilation, and analysis of clinical program activity data.
6.    Provides technical assistance, resource services, consultation and education to clinical departments relative to core measures activities.
7.    Participates in and conducts process improvement analysis where applicable.
8.    Responsible for extension of the program in terms of new services or areas.
9.    Promotes an interdisciplinary approach in patient care delivery.
10.    Serves as a resource for patients and their families.
11.    Acts as a patient advocate utilizing the Patient Bill of Rights.
12.    Participates in promoting and maintaining a safe and therapeutic environment.

Administrative Practice
1.    Prepares reports and analysis associated with the institutions core measures programs i.e. senior management reports, departmental and clinical service program evaluation, quarterly reports, etc.
2.    Facilitates cooperation within the program between all departments, services and units.
3.    Provides timely status reports of results of quality monitoring relative to areas that are assigned including identification of patterns and trends that require actions.
4.    Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
5.    Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits.
6.    Participates in the development and implementation of continuous quality improvement programs.
7.    Participates in quality management activities including identifying and communicating issues of performance improvement and risk in a timely manner.
8.    Participates in committee activities.
9.    Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.
10.    Recognizes each individual's right to health care unrestricted by age, race, color, sex, age, national origin, disability, sexual orientation or source of payment.
11.    Assists in the performance appraisal process of designated Nursing staff.
12.    Researches opportunities for use in benchmarking for assigned areas.
 


Qualifications
  • Baccalaureate Degree in Nursing is required a Master?s in Nursing is required 
  • Progressive job-related experience specific to area of responsibility
  • New York State License with current registration as a registered professional nurse, nurse practitioner preferred.

PURPOSE AND SCOPE: Ensure the provision of quality patient care to all hospitalized patients in the most efficient manner in accordance with company policy. Provides direct supervision and coordination of inpatient staff. Responsible for high volume monthly treatments. Utilizes the FMC mission, core values and customer service philosophy to ensure quality service while maintaining a high level of efficiency management. PRINCIPAL DUTIES AND RESPONSIBILITIES: Manages the tactical execution of short- and long-term objectives through the coordination of activities with a direct responsibility for results, including costs, methods, and staffing. Ensures all acute treatments are performed in contracted hospitals according to the established policies and procedures and in compliance with all applicable regulatory requirements. You will work in a growing, specialized area of the nursing field outside the walls of a hospital setting As a Home Infusion Registered Nurse you will provide a broad range of infusion therapies in out patients’ homes while: Compassionately taking care of patients in order to allow them to recover in the comfort of their own home by using your clinical IV capabilities and personal touch. Skillfully providing one-on-one home infusion nursing care visits and patient education in accordance with the physician’s plan of treatment. Assessing the patient’s condition and response to therapy, taking action as needed to facilitate achievement of treatment goals. Establishing trust and building therapeutically effective relationships with our clients and their families. Required Qualifications: Active and unrestricted registered nurse with current license in state of NY Two (2)+ years of experience working as an RN in a Critical Care setting that contributes to proficiency in infusion therapy (such as medical surgical unit, ED, ICU, post-anesthesia care, or similar setting) Experience with lab draws, IV infusions, and reconstitution of medications Strong experience with working with a variety of pumps (CAAD, curling, etc) & troubleshooting issues that may arise Possess a valid and current driver’s license and auto insurance/registration Ability to handle specialty cases like IGG, IVIG, Solumedrol, etc. Have and maintain current CPR / BLS certification Knowledge of HIPAA patient privacy and data security requirements; OSHA employee protection regulations Hold current Malpractice Insurance Have experience with Microsoft Office to document and track patient care activity About Us: Founded in 2008, UniversaCare is a fully licensed and growing Home Care Agency serving the NY Metropolitan area and is continually expanding throughout the Northeast.
