RN Supervisor - VNA - Full Time - Days

Job Summary

The Nursing Supervisor is responsible for the oversight and coordination of patient care services.  Through direct supervision and coordination, the Nursing Supervisor will ensure safe and therapeutic care is provided to the patients of Visiting Nurse Association of Utica and Oneida County (VNA)  in compliance with New York State (NYS) Department of Health (DOH) licensure guidelines, the Nurse Practice Act, agency policies and procedures, and industry established best practices.   The Nursing Supervisor provides direct supervision to the clinical staff providing care in the home.

Core Job Responsibilities

  • Coordinates referrals of new patients and the assignment of personnel to each case.
  • Maintains staffing schedule to assure 24 hour per day, seven days per week coverage for patient care needs.
  • Provides oversight of clinical staff to assure accurate, continual assessment of patient care needs and confirms documentation of assessment findings is done in timely manner. 
  • Coordinates the development of an individualized interdisciplinary plan of care for patients and ensures the implementation of the plan with regular updates.
  • Leads interdisciplinary team meetings to enable the review and update of the patient’s plan of care.
  • Completes assigned quality record audits per agency policy, as well as auditing records when patient care issues are identified.
  • Has a working knowledge of agency data collection software. 
  • Completes supervisory home visits with licensed staff to ensure competency in skill set to educate new staff to the role of a home care nurse and to act as a resource in patient care.
  •  Completes reviews and performance appraisals of their direct report staff according to agency policy and procedure and when applicable applies disciplinary measures as appropriate.
  • Has a strong working knowledge of home health regulations as it pertains to outcome measure and utilization of services to ensure that industry best practices are employed and quality patient outcomes are met. 
  • Acts as a resource to providers to educate and facilitate agency services to the community of Oneida County.  This may include the need to participate in off sight patient center conferences. 
  • Participates in developing manuals, policies, and informational materials for use by the nursing staff.
  • Has a working knowledge of the Electronic Medical Record (EMR) system in use by VNA and ensure staff proficiencies with use of the system as per the individual job description.
  • Completes nursing visits to patients per agency need.
  • Completes supervisory on-call and weekend rotation as assigned to ensure twenty four hour coverage.
  • Complete other duties as assigned.
     

Education/Experience Requirements

Required:

  • Baccalaureate degree in Nursing or a health or human services field,
  • Two (2) years’ experience in home care

or a combination of

  • five (5) years’ experience in home care;
  • six (6) credit hours, or the equivalent, of education/training in public health and principles of management. 


 

Licensure/Certification Requirements

Required:

  • Licensed and currently Registered Professional Nurse
  • Current Basic Life Support (BLS) Certification
  • NYS Driver's License
