Clinical Program Manager-Geriatric and Palliative Medicine

The Clinical Program Manager for ALIGN-CARE Geriatric Surgery Co-Management Program is responsible for overseeing the continuity of patient care. Patients will be 65 years or older, frail, community-based, considering surgery, assessing surgical risks and provide care plans to optimize health prior to surgery. Patients enrolled in the program may come from any part of Mount Sinai Health System; however, hospitals of care receipt will be both Mount Sinai Hospital and Mount Sinai Morningside Hospital.

• Oversees clinical operations within the program, using data and fact-based problem-solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 
• Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes or recommends changes to superiors as necessary.
• Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities.
• Serves as a clinical role model and resource to the nursing staff. May include direct patient care/clinical duties including patient assessments, testing, patient education, and managing patient emergencies.
• Provides timely status reports of results of assignments including identification of patterns and trends that require actions.
• Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
• Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits.
• Responsible for components for team-based initial assessment of patients
• Responsible for follow-up tracking and phone calls for education and counseling prehospitalization, peri-operatively, and post operatively
• Will follow discharged patients from hospital via telephone and transitional care management workflow for optimal post-operative care with the mission to reduce unnecessary return to the emergency room or hospitalization
• Responsible for tracking patients throughout the program (Preoperative, perioperative and postoperatively) in a secure patient database
• Documentation of clinical encounters in EpicCare EMR, providing patients and caregiver instruction and coaching, and corresponding with surgical teams (Surgeon, Physician Assistant, Nurse Practitioner, Administrative Assistants) and extended care team (Physical Therapist, Visiting Nurses, Pharmacists, etc.) as needed
• Telephonic and video visit (virtual) care and from time to time in-person care (outpatient clinics at MSH and MS Morningside sites).
• May also work with medical trainees (i.e. medical students or geriatric medicine fellows)
• As part of the evaluation and development of the program, the CPM will oversee collection, compilation and analysis of clinical program activity data to evaluate clinical performance and competence or the program and its development

• Responsible for generating biweekly and monthly reports of program progress, performance, specific to each surgical team and primary care provider, and presenting reports to stakeholders and team members as needed to facilitate ALIGN-CARE program growth, research and development.

   

• Baccalaureate Degree in Nursing is required. Masters in Nursing or health related field is preferred. Certification in Gerontology or older patient care specialty area preferred.
• 5 years directly related experience (some supervisory preferred)
  New York State License with current registration as a registered professional nurse. Certification in specialty area preferred.

Compensation

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $115494 - $192328 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
