UChicago Medicine is seeking a Nurse Associate to join the Ambulatory Float Pool team, supporting multiple outpatient clinics in Hyde Park and surrounding locations. This role provides the opportunity to deliver high-quality patient care across a variety of specialties, including Cardiology, Neurology, Oncology, Primary Care, Geriatrics, Pediatrics, and more. 

 

As a Clinic RN, you will provide patient services in the clinical and research area. You will perform a wide variety of nursing care services to patients under the direction of the Clinical Nursing Supervisor, Physician Director of the practice, and/or Director of Outpatient Operations. Such services can be therapeutic or diagnostic procedures and/or administration of medication. You may participate in all aspects of research clinical trial administration, including: screening, recruitment and interviewing of subject, collection of data from hospital records, telephone follow-up with subjects, and some data entry.

 

Essential Job Functions

  • Performs any or all duties involving direct patient care as assigned by the Clinical Nursing Manager nurse and directed by Nurse Practice Act and Hospital and Patient Services Sector policies and procedures
  • Utilizes the educational assessment of the patient and family which is performed by the R.N. to appropriately implement, evaluate, and document patient and family education
  • Observes and records vital signs, assesses patient reactions and reports changes to Licensed Medical Physician
  • Assists the physician with patient examinations or diagnostic procedures
  • Collaborates with the Licensed Medical Physician in formulating plans of care for assigned patients. Interacts with multidisciplinary team members to positively influence patient centered care and focus on problem solving. Works with the patient care team to carry out the agreed upon plan of care
  • Administers medication in accordance with policy and procedure utilizing knowledge base to assure patient safety
  • Is accountable for communicating all changes in the patient’s biophysical, emotional, and spiritual status to the Licensed Medical Physician
  • Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical and psychological needs of neonatal, pediatric, adolescent, and geriatric patients
  • Consistently communicates with registered nurse, multidisciplinary team members and physician to enhance quality of patient care and unit activities

 

Required Qualifications

  • Currently licensed as a registered professional nurse in the State of Illinois
  • Associate Degree in Nursing; BSN degree or higher preferred.
    • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
  • Two to three years’ experience as a registered nurse

 

Preferred Qualifications

  • 2 years’ previous Ambulatory experience 
  • Pediatric Experience
  • Computer proficiency. Familiarity with UCH Information Systems (Epic, OACIS) 
  • Strong interpersonal skills; ability to handle a wide variety of public contacts, including conflicts, with patience, tact, courtesy and discretion
  • Pediatric experience

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.

MDS Coordinator

SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law. Other duties as assigned.
