Licensed Practical Nurse (LPN) Job Summary:

As the Homecare Licensed Practical Nurse (LPN), you will perform selected nursing care for Patients in Care of Sick, and/or Hospice Programs as delegated by the PCN whom works in a nursing team relationship, under general supervision of the Clinical Supervisor.

Licensed Practical Nurse (LPN) Job Responsibilities:

  • Collaborates with PCN to review Plan of Care and provide nursing care to patients in their homes on a family centered basis while recording/maintaining complete, accurate patient record on Portable Electronic Device and/or via paper record.
  • Makes patient assessment, reports and documents appropriately as delegated by the registered nurse within their scope of practice.
  • Works under direction of PCN in orientating and delegating responsibility to the Home Health Aides within Regulatory Guidelines as per agency policy.


Licensed Practical Nurse (LPN) Education and Experience Requirements:

  • LPN Degree required.
  • Current CT license required.
  • Minimum of four (4) years of nursing practice, two (2) years of which must be recent Med-Surg, acute care, or critical care experience as a Licensed Practical Nurse preferred.
  • Valid Connecticut Driver’s License and proof of current insurance coverage required
