Tempositions Health Care Solutions is seeking a hematology nurse practitioner or physician assistant for a long-established practice with 8 physicians and 4 Advanced Practice Providers.

Qualification Requirements: 

  • Master’s degree from an accredited school of nursing or physician assistant  
  • Eligible for or currently licensed APRN/PA in state of CT 
  • Maintains continuing education credits and license renewal 
  • ONS certification for APRN required or to be obtained within 2 years of employment 

Preferred Qualifications: 

  • Minimum of 2 years previous experience in a similar position 
  • Oncology experience is major plus 

Working days: Monday -Friday (4 days per week)

Nurse Practitioner - Advanced Heart Failure Services - Queens Medical Group As part of the Advanced Heart Failure team, the Nurse Practitioner will play a central role in both inpatient and outpatient care delivery, helping improve transitions of care and long-term outcomes for this high-risk patient population. Duties will include but not limited to the following: Key Responsibilities Provide comprehensive care to advanced heart failure patients in collaboration with heart failure cardiologists and the multidisciplinary team Participate in both outpatient clinic care and inpatient service responsibilities Conduct inpatient consultations for the Advanced Heart Failure service Facilitate outpatient management, including: Telephone triage of patient symptoms Review and follow-up of diagnostic results and lab findings Medication titration and care coordination Support transitions of care between hospital discharge and outpatient follow-up Participate in the evaluation and workup of patients for advanced heart failure therapies (including mechanical circulatory support and transplant evaluation as applicable) Provide care for patients with advanced heart failure devices, including management of patients with remote pulmonary artery pressure monitoring systems Deliver care in accordance with Practice Protocols, Clinical Practice Agreements, and institutional policies This is a 4 days per week, day shift position based in Flushing, NY, with responsibilities across both inpatient and outpatient settings.
