Monitors patient progress and symptom severity by conducting a structured telephonic interview and using validated tools (e.g. PHQ-9, GAD-7 and PCL-5). Provides patient education as an integral part of the services offered (e.g. medication education, alcohol effects on mood, etc.). Collaborates with patients to develop SMART self-management goals to assist patients with symptom improvement. Assists patients with treatment plan adherence. Communicates with providers regarding patient status and documents appropriately in the electronic medical record. Recognizes and reports abnormal test results, changes in symptoms and the need for follow-up. Communicates at least weekly with BHC and bi-weekly with the External Mental Health Consultant (EBHC) to review patients that require staffing.
