Program Overview
This position focuses on community-based care for justice-involved individuals. The program provides temporary housing and clinical services for individuals on parole who have serious mental illness and substance use disorders. The goal is to support clients in stabilizing their health and successfully transitioning to long-term housing and ongoing community-based care.
Position Summary
The Registered Nurse (RN) provides on-site nursing care under the supervision of the Nurse Manager. This role focuses on supporting clients’ medical needs, coordinating care, and promoting overall health and stability.
Qualifications
- Active New York State RN license.
- Experience working with homeless or vulnerable populations is a plus.
- Basic computer skills (Microsoft Word, Excel, and electronic medical records; eClinicalWorks is a plus)
