Job Details Description Position Summary: A team-based care RN (Registered Nurse) navigator plays a pivotal role in delivering coordinated, patient-centered healthcare within a multidisciplinary team. Their responsibilities emphasize collaboration, evidence-based practice, and quality improvement. The RN Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients. The Navigator proactively identifies and resolves patient’s needs. and clinical guidance throughout their care within Deborah Heart and Lung Center and it's practice locations. The RN navigator coordinates a plan of care across the system for Deborah patients. The RN Navigator systematically and continuously collects and assesses information related to patient health status to develop, execute, and evaluate the plan of care for patients and/or family.
