In this role, you will be part of a team of enthusiastic and knowledgeable Wound, Ostomy Care Specialists. Each member of the team operates on a high level of autonomy and is responsible for delivery of patient care and consultations for patients with wounds and ostomies. WOC nurses collaborate with unit leadership and staff RNs by recommending appropriate treatment and prevention plans, while coaching, and mentoring staff in wound and ostomy care. Through our Skin Champions council, our team supports and further mentors staff RNs who have a vested interest in preventing pressure injuries on their respective units. WOC nurses are given a seat at the decision making table and we participate in policy and product review regularly to ensure that practice is evidence based. You will regularly evaluate patient outcomes, and participate in initiatives focused on improving quality and performance. Our team collaborates on all aspects of care and although we promote individual practice, we work together with the common goal to promote patient safety.

This is a per diem position (8AM - 6PM) located at New York-Presbyterian Brooklyn Methodist Hospital.

Preferred Criteria

Master's in Nursing or related field
Experience in wound, ostomy, and continence management

Required Criteria

Bachelor's Degree in Nursing (BSN)
Current New York State RN license
At least 3 years of acute care RN experience in Medical Surgical
Wound, Ostomy and Continence Certification (CWON)
CWOCN certification
BLS and ACLS certifications from the American Heart Association
Excellent customer service and communication skills

Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects. Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress. Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home. Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes. Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient. Documentation : Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses. Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care. QUALIFICATIONS Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university Currently licensed, in good standing, to practice in New York as a Registered Nurse. Minimum 1 year community health nursing experience or equivalent professional experience. Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS. Complies with accepted professional standards and practice.
