JOB DESCRIPTION

Accountabilities

  • Budget Responsibility: Assists management with maintaining par inventory levels in a fiscally responsible manner.
  • Authority/Decision Making: Takes appropriate clinical actions using available resources to achieve positive outcomes for patients, visitors and staff. Independently prioritizes daily tasks and assignments. Identifies and recommends opportunities for improvement in order to ensure patient safety and quality outcomes. Response to urgent and unplanned situations appropriately.
  • Problem Solving: Capable of making decisions to ensure patient safety and promote quality patient care.
  • Information Management: Utilizes computer systems as required.
  • The Registered Nurse (RN) in the ambulatory practices functions as an integral member of the interdisciplinary healthcare team in providing patient care.
  • The RN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care.
  • The RN utilizes evidence to drive clinical decisions and practice related to individual and population health.
  • The RN considers factors related to safety and effectiveness in planning and delivering care.
  • The RN provides care in accordance with the NJ Nurse Practice Act.
  • The RN must successfully pass all required training to obtain security access to perform role, including EMR training, AMG clinical training, department specific clinical competencies, among others.
  • Will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served.
  • Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
  • Must have excellent communication, organization, and computer skills.
  • Must have the ability to work in a highly specialized area with a focus on patient safety and efficiency.


Responsibilities

  • Triage and assess patient calls and electronic messages related to vascular symptoms, including claudication, limb pain, edema, non-healing wounds, and post-procedural concerns
  • Provide clinical guidance and patient education regarding vascular disease processes, treatment plans, medications, and lifestyle modifications
  • Manage high-volume in-basket tasks, ensuring timely response to patient inquiries and coordination of care
  • Review diagnostic results (e.g., ABI, duplex ultrasound, CTA/MRA) and facilitate appropriate follow-up per provider direction
  • Coordinate pre- and post-procedure care for vascular interventions, including angiograms, endovascular procedures, and surgical follow-ups
  • Conduct pre-operative chart reviews, verifying history, imaging, labs, medications, and required clearances to support safe and efficient surgical care
  • Monitor post-operative patients for complications such as bleeding, infection, access-site issues, or changes in neurovascular status
  • Process medication refill requests, including antiplatelets, anticoagulants, statins, and antihypertensives, following clinical protocols
  • Collaborate with providers to prioritize urgent vs routine clinical issues using sound nursing judgment
  • Support care transitions following hospitalizations, procedures, or emergency visits to ensure continuity of care
  • Document all clinical assessments, interventions, and patient communications accurately within the electronic health record
  • Collaborate with multidisciplinary teams, including surgeons, advanced practice providers, wound care specialists, and vascular labs
  • Ensure compliance with clinical protocols, quality measures, and patient safety standards
  • Assist with in-office clinical operations during scheduled office hours as needed, including patient intake, vital signs, rooming, and provider support
  • Assist with completion and coordination of disability forms, FMLA paperwork, and insurance forms as necessary to support continuity of care and compliance.


Required

QUALIFICATIONS

  • High School diploma required
  • Graduate of an accredited Nursing Program required
  • Valid NJ RN License


Preferred

  • BSN highly preferred
  • 2-3 years of clinical experience in cardiovascular, vascular surgery, cardiology, or related specialty
  • 2-3 years of clinical experience in an acute care setting, preferably critical care
  • BLS certification from American Heart Association
