ChristianaCare is currently seeking seeking a dedicated and compassionate Memory Care Specialist APC to join our team. This role can be filled by a candidate with experience in Geriatrics/FNP, Psychiatry, or Neurology with a passion for providing exceptional care to patients with memory disorders, including Alzheimer’s disease and other forms of dementia. The ideal candidate will have a strong background in diagnosing, treating, and managing cognitive impairments and will work collaboratively with a multidisciplinary team to enhance the quality of life for our patients.  Candidates with specialty/fellowship training are encouraged to apply.

**This position will support the Swank Urgent Care and Inboxologist patients.

Work Schedule:

Monday - Friday, day shift, no weekends, holidays, or on-call required

Key Responsibilities:

  • Conduct comprehensive assessments and evaluations of patients with memory disorders.

  • Develop and implement individualized care plans tailored to the needs of each patient.

  • Provide ongoing medical management and follow-up care for patients with cognitive impairments.

  • Collaborate with other healthcare professionals, including nurses, social workers, and therapists, to ensure a holistic approach to patient care.

  • Educate patients and their families about memory disorders, treatment options, and strategies for managing symptoms.

  • Participate in clinical research and stay updated on the latest advancements in memory care.

  • Maintain accurate and detailed medical records in compliance with healthcare regulations.

  • Provide leadership and guidance to the memory care team.

Qualifications:

  • Masters degree (NP or PA) from an accredited institution.

  • Board certification in Geriatrics, Psychiatry, or Neurology.

  • Valid license to practice in DE.

  • Strong interpersonal and communication skills.

  • Compassionate and patient-centered approach to care.

  • Ability to work effectively in a multidisciplinary team environment.

  • Commitment to ongoing professional development and education.

Annual Compensation Range $112,554.00 - $139,477.00

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
