The Licensed Practical Nurse (LPN) – Home Health is responsible for providing skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN). This role includes assessing, planning, implementing, and evaluating patient needs to ensure high‑quality, compassionate care.
Position Details:
Work Schedule
- Standard Hours: Monday–Friday, 8:00 AM – 4:30 PM
- Weekend Call: Required twice per month
Minimum Qualifications:
Licensure:
- Graduate of an accredited practical nursing program.
- Must hold and maintain a current LPN license in the hiring state, meeting all State Board of Nursing requirements.
- Must possess a valid driver's license.
Experience:
- Minimum of 1 year of nursing experience.
Certifications:
- CPR certification required
Preferred Qualifications
- LPN‑IV Certification (if applicable).
- Additional unit‑specific certifications as required by department or obtained within designated timeframes
Physical Requirements
- Ability to push, pull, and/or lift up to 50 lbs. regularly.
- Ability to stand and walk for prolonged periods.
- Ability to grip, reach, bend, kneel, twist, and squat to perform job duties.
