Job Description Summary: The Advance Practice Nurse (APN) provides psychiatric care to patients. APN provides services to individuals impacted by a variety of behavioral health (addiction/substance abuse and mental health) issues, recognizing that the individuals we serve often have experienced trauma that affects their development and adjustment. APN demonstrates a warm and welcoming empathic, hopeful attitude, conveying a philosophy of dual recovery. APN will be trained in trauma-informed care and will work to help provide services in an environment that is sensitive and responsive which will prevent victimization, abuse, or trauma as a result of the care received. JOB SUMMARY: The LPN will be responsible for greeting patients, activating patient files and providing support to patients and medical staff. The main difference in between the Cardiac department and any other medical floor is that we see a few more specialized patients in addition to the typical medical population. For example, we take post op CABG/open heart patients as soon as day 1 following surgery, TAVRs, Watchmen and other post cath-lab intervention patients (stents, AIF, etc.) but they will never have a sheath, they may come to us with a radial band. None of our patients are on vasoactive drips but we do take amio, Cardizem and other cardiac drips and do titrate down to wean but don’t increase the rate on our floor. We don’t do nitro drips. Many of our patients have chest tubes. Acuity is really varied, some patients are simply needing their medications adjusted and must be supervised in the hospital with serial EKGs but otherwise are completely independent, some of our patients are very complex and sick but based on our treatments/interventions are still appropriate for our floor. Qualifications: Education: Graduated from a school of nursing (Associate's Degree, Diploma, or BSN). Licensure: Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements. Additional Perks & Benefits - Scroll down to bottom of this post to learn more Key Responsibilities: - Perform all appropriate nursing assessments per service line requirements, utilizing a high level of clinical competence. - Evaluate the client/patient’s physical condition and the need for services. - Complete required screenings, record the client/patient’s initial medical histories and vital signs; coordinate any required action planning based on results. - Monitor the health status of the client/patient during services provided. - Work with the treatment team to coordinate care based on client/patient needs. - Create action plans for client/patient based on physical and mental health in regards to treatment needs. - Oversee infectious disease control, client/patient hygiene, and related client & facility health issues, under the supervision of nursing leadership. - Provide disease prevention, risk reduction, and other health education as needed. - Arrange medical appointments for client/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions. - Consult with a physician/advanced practice provider as necessary or advocate for medical services through managed care organizations. - Arrange or monitor special dietary needs for medical conditions. - Review medication requirements with clients/patients, educating the client and guardian (if necessary) about the benefits of taking prescribed medications as prescribed. - Administer and oversee administration of medications via various methods according to service line requirements; Monitor medication compliance in regard to State Regulatory standards. - Consult with the physician/advanced practice provider or pharmacy to confirm medications prescribed. - Oversee therapeutic injection of medication (subcutaneous or intramuscular). - Monitor lab levels including consultation with physicians/advanced practice providers, clients, and clinical staff. - Coordinate medication needs with pharmacies, clients/patients, and families, including the use of indigent drug programs. - Monitor medication side effects including the use of standardized evaluations. - Monitor physician orders for treatment modifications requiring client/patient education. - Assist Physician/Advanced Practice Provider with client/patient exams. - Set up and draw any necessary labs and perform any special procedures as requested by the Physician/Advanced Practice Provider. - Complete all required documentation in a timely manner as indicated in company policies and procedures. - Demonstrate interest in long-term and short-term goals and objectives of the company. - Other duties as assigned by program leadership. Experience and/or Education Qualifications: - Current RN (Registered Nurse) license in Missouri. - Current LPN (Licensed Practical Nurse) license in Missouri. Employment Requirements: - Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check. - Completion of New Hire Orientation at the beginning of employment. - All training requirements including Relias at the beginning of employment and annually thereafter. - Current driver’s license, acceptable driving record, and current auto insurance.
