Home care provider serving Northeast Georgia. Seeking a motivated, compassionate, and experienced CNA/LPN to join our team and serve as the cornerstone of our client and caregiver experience.

KEY RESPONSIBILITIES
Client Relationship Management:
Serve as a point of contact for clients and their families from intake through ongoing care and maintain communication
Develop individualized care plans in collaboration with the client and family based on in-home assessment and maintain the plan as needs change
Caregiver Relationship & Staffing:
Build strong, respectful relationships with caregivers — understanding their strengths, preferences, and availability
Coordinate and manage shift schedules, ensuring consistent coverage and appropriate client-caregiver matching
Organic Growth & Community Presence:
Identify opportunities to expand services needs within the existing client base as they evolve
Build and maintain relationships with referral sources: rehab facilities, physicians, discharge planners, senior living communities, and social workers throughout Hall County and NE Georgia
Represent the agency at community & marketing events and senior center programs

QUALIFICATIONS

Minimum 2–3 years of experience in home care, assisted living, memory care, skilled nursing, or a comparable senior care setting
Strong interpersonal and communication skills — both verbal and written; comfortable with families, seniors, and clinical professionals, strong desire for teamwork
Demonstrated ability to manage relationships with multiple stakeholders simultaneously
Scheduling and organizational competence with the ability to handle shifting priorities calmly and effectively
Preferred / Differentiating
Existing referral network in Hall County or surrounding NE Georgia care communities
Prior experience in a client-facing role within healthcare or senior services
