Position Summary: 

The primary purpose of the position is to complete the Resident Assessment Instrument (“RAI”) process in accordance with the requirements of State and Federal law as well as the policies and goals of this Community. This is not a remote position.

 Education/Qualifications: 

  • Active, unencumbered Registered Nurse license in the State of practice.  

  • Must have an active CPR or BLS Certification. 

  • Must possess intermediate computer skills and ability to navigate through various software programs, including electronic health record software.

  • Previous experience required.


Shifts & Wages:

  • Wage: $50.00 per hour

We are looking for an experienced regional MDS Coordinator to join our healthcare facility! Key Responsibilities: Provide expert guidance and oversight for the completion and accuracy of MDS assessments across the region. Ensure compliance with CMS regulations, including Resident Assessment Instrument (RAI) guidelines. Review MDS documentation to ensure it supports care plans and reimbursement. Train and mentor facility-level MDS coordinators and interdisciplinary teams on MDS processes, RAI guidelines, and changes in regulations. This position requires for work to be completed at the facility. This is not a remote position. This position pays up to $90,000 per year depending upon experience. Job Responsibilities: Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and Facility policies. Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification. Assists with ensuring that documentation at the facility meets Federal, State, and Certification guidelines. Coordinate the RAI process assuring the timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Monitor the facility's QI and QM report. Develop the schedule of activities required for the resident assessment and plan of care. Inform all assessment team members of the requirements for accuracy and completion of the resident assessment (MDS). Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS. Job Requirements Registered Nurse with a current License in the state of PA.
