Schedule – Weekend Baylor Program
The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes.
The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards.

Schedule – Weekend Baylor Program
- 24 hours/week - Friday, Saturday, Sunday
- 8:00 AM – 4:30 PM Enjoy a predictable schedule while making a meaningful impact during critical weekend coverage hours.
The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes.
The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards.
Key Responsibilities
- Perform comprehensive patient assessments in the home setting
- Develop and implement individualized care plans and goals
- Provide skilled nursing care and treatments according to physician orders
- Educate patients and families on disease management, medications, and care plans
- Monitor patient progress and adjust care plans as needed
- Document care accurately in the electronic medical record
- Collaborate with physicians, therapists, and care team members to coordinate care
- Maintain patient confidentiality and adhere to professional and regulatory standards
Minimum Qualifications:
- Graduate of an accredited professional nursing program required
- Bachelor of Science in Nursing (BSN) preferred
- Minimum two (2) years of medical/surgical nursing experience preferred
- Experience or knowledge in community health or home health nursing preferred
- Current Registered Nurse (RN) license in Massachusetts required
- BLS Basic Life Support (Healthcare Provider) certification required
- Strong clinical nursing and patient assessment skills
- Excellent communication and patient education abilities
- Critical thinking and sound clinical judgment
- Effective time management and organizational skills
- Proficiency with electronic medical records and computer systems
- Commitment to maintaining patient confidentiality
- Ability to read, write, and communicate effectively in English
- Demonstrated customer service and patient-centered care skills
