We're looking for an Admissions & Discharge Nurse (RN) to join our leadership team. In this vital role, you'll help ensure every resident experiences a smooth, safe transition into and out of our facility while delivering exceptional clinical care and customer service.

Preferred Schedule:
12:00 PM – 8:00 PM
1:00 PM – 9:00 PM
2:00 PM – 10:00 PM
Per diem applicants are also encouraged to apply while we recruit for a permanent team member.
What You'll Do
As our Admissions & Discharge Nurse, you'll play a key role in creating a positive experience for residents and families by ensuring seamless admissions and safe, efficient discharges.
Responsibilities include:
Conduct comprehensive nursing assessments for new admissions.
Review medical histories and collaborate with interdisciplinary teams to develop individualized care plans.
Coordinate admissions and discharges with physicians, hospitals, residents, families, and healthcare providers.
Educate residents and families about services, care expectations, and discharge planning.
Assess discharge needs and coordinate home care services, equipment, and follow-up appointments.
Ensure compliance with all federal, state, and facility admission and discharge requirements.
Maintain accurate clinical documentation and required reports.
Participate in interdisciplinary care planning meetings.
Serve as a clinical resource regarding admission and discharge planning.
Perform other related duties as assigned.
