Bridgewater Center for Rehabilitation and Nursing is seeking compassionate and caring Licensed Practical Nurses (LPNs) to join our team on our overnight shift (10:30PM-6:30AM). Responsibilities:

Provide direct patient care to residents on our Long Term Care units and specialty skilled areas
Implement individualized care plans under the supervision of the registered nurse
Monitor and report changes in residents' conditions to the nursing team
Participate in medication administration and management
Collaborate with other healthcare professionals to ensure effective care delivery
Maintain accurate and up-to-date records of patient care
