We are offering a $3,000 sign-on bonus for full-time Registered Nurses joining our Second or Third Shift teams!

The Citadel of Sterling is seeking a Registered Nurse (RN) to join their team! As a Registered Nurse (RN) you will be assuming responsibility and accountability for a group of residents for or a shift of duty. Nursing care is provided through assessment, implementation, and evaluation of the plan of care. 

Registered Nurse (RN) Duties and Responsibilities:

  • Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations 

  • Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner.

  • Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. 

  • Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. 

  • Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. 

  • Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care 

  • Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. 

  • Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. 

  • Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance. 

  • Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance.

  • All other duties, as assigned.

Registered Nurse (RN) Qualifications and Skills:

  • Excellent written and verbal communication skills in English.

  • Ability to organize and prioritize work. 

  • Understands and follows infection control standards, policies and procedures.

  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.

  • Possess special interest in, and a positive attitude about, working with short and long-term care residents and the elderly.

  • Meets all health requirements, as required by law.

  • Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Microsoft Office version 1997, 2000 or XP, MS Word, MS Access, MS Excel, E-mail, Outlook, Internet Explorer. 

Registered Nurse (RN) Education and Experience:

  • Nursing Degree or diploma from an accredited College or University. 

  • Possess a current license to practice in the State as a Registered Nurse. 

  • 1 year of experience preferred.

  • Long-Term care experience preferred.

Physical Requirements: 

  • Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. 

  • Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required. 

  • Meet all health requirements as needed.

Overview The Case Manager leads care coordination throughout hospital levels of care. In this role the Case Manager completes assessments and transition care planning that reflects the wishes and needs of the patient and their family. This position also assists physicians and finance with the management of resource utilization through the use of medical necessity criteria. Responsibilities Completes medical necessity criteria for both inpatient and outpatient visits, documenting the reasons when the patient status is not supported in the database and reporting discrepancies during weekday morning huddles. Completes assessments and discharge plans for inpatients. Provides consultation for outpatient and observation patient stays when requested. Assists with discharge planning for these patients as needed. Builds provider networks and community resources and provides updates to the clinical team. Assists in the case management functions of the preoperative preparation and discharge planning for Inpatient or outpatient surgeries, specifically the Total Joint Program as able. Leads morning huddles focusing on patient status, expected LOS and providing the tentative plan for discharge. Completes LACE tool for Inpatients and reports Lace scores to hospitalist group for furthering discussions regarding palliative care and readmission reduction. Role models values and actively applies patient engagement strategies; documents information on the white boards including what matters to the patient. Completes the MY Plan for all inpatients and uses the "journey" program to provide discharge planning education to patients and their families. Completes follow-up phone calls for high risk patients as time allows. Assists with requests for clinical information for medical necessity insurance reviews and arranges peer to peer calls when an appeal is warranted.
