The Registered Nurse will plan, organize and direct hospice care. They will also build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position Responsibilities:
- Coordinating the care of the patients assigned to you.
- Making regular visits to patients' homes to assess patients' physical, emotional, spiritual, and social needs.
- Working collaboratively with the hospice physician and your supporting LVNs to carry out appropriate interventions.
- Helping the agency to continuously increase the quality of care.

Position Requirements:
- Licensed as a Registered Nurse/RN
- Valid Driver's License and reliable vehicle with current insurance.
- Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
- CPR Certified.

Hospice LVN Duties Include:

-Service a range of patients and family during transition of life

-Ensure quality and safe delivery of Hospice services

-Implements current nursing practices and provide direct hands-on care within scope of practice

-Obtains orders as needed to promote comfort and quality of life

-Keep Case Manager informed of all changes in condition and orders obtained
