Visiting Nurse & Hospice of Litchfield County Winchester, CT, US

Visiting Nurse & Hospice of Litchfield County is seeking an RN with experience in hospice & palliative care to join our progressive team to cover the Salisbury, Lakeville, Sharon area.

Salaried full-time: 37.5 hours per week. Schedule: Monday - Friday. Weekend rotation is one weekend every 7 weeks and one holiday per year. Recent experience in the home care setting required.

Per Diem Case Manager: per diem position requires coverage one weekend per month and one holiday per year.

The Agency utilizes Netsmart for its EMR and adheres to National Best Practice Guidelines for caseload and visit productivity.
