Position: School Nurse/Health Service Supervisor Location: Granby Public Schools (Elementary, Intermediate, Middle, and High School) Organization: Visiting Nurse & Hospice of Litchfield County (VNHLC) Schedule: Hours - 30 hours/week. Ten-Month Position (August – June) Overview: VNHLC manages the School Nursing Program for the Town of Granby, ensuring high-quality health services across all public-school levels. We are currently seeking a qualified and compassionate School Nurse/Health Service Supervisor to oversee and support nursing services throughout the Granby Public Schools. Key Responsibilities: Supervise and support school nursing staff in daily operations and student health services. Oversee the delivery of comprehensive healthcare services and ensure compliance with school health regulations and policies. Collaborate with school administrators, teachers, parents, and healthcare providers to support student well-being and manage health plans. Provide direct care as needed, including emergency response, health assessments, and chronic disease management. Maintain accurate health records and ensure confidentiality in accordance with HIPAA and FERPA. Participate in health education, wellness initiatives, and professional development for school nurses. Requirements: Current, valid Registered Nurse (RN) license from the State of Connecticut. Current BLS or CPR/AED certification. Bachelor’s degree in nursing or a related field (preferred). Visiting Nurse & Hospice of Litchfield County is currently looking for a Home Health Aide/Certified Nursing Assistant to join their team. The ideal candidate should possess a minimum of 1-2 years Home Health Aide experience working for a licensed and Medicare Certified Home Health or Hospice agency. Responsibilities include: making visits to patient homes for both home health and hospice to provide personal care activities including bathing, oral hygiene, feeding, dressing, and assisting the patient with ambulation and transfer activities. Must be flexible, reliable, and possess good organizational and communication skills. Positions available in both Litchfield County and the Greater Farmington Valley.
Licensed Practical Nurse (LPN) - Per Diem Visiting Nurse & Hospice of Litchfield County is seeking experienced Per Diem LPNs to join our clinical teams serving Litchfield County and the Greater Farmington Valley. Must have availability for weekends and one holiday per year. Duties include performing home visits; observe, document, and report patient status; and coordinate care with multi-disciplinary team. The Agency utilizes Netsmart for its EMR and follows National Best Practice Guidelines for caseload and visit productivity. We are in need of CT-licensed RN for per diem school nursing (Substitute School Nurse) coverage within the Granby Public Schools. This position offers flexible hours, with both full and partial days. Candidate must hold a current, valid State of Connecticut RN license and current BLS or CPR/AED certification. A Bachelor's Degree and at least one year of experience in pediatrics or school nursing is preferred.
