Job Summary

We are seeking a dedicated and compassionate MDS Coordinator to join our team at Northwood Skilled Nursing and Rehabilitation in Springfield, Ohio. As an MDS Coordinator, you will play a vital role in ensuring our residents receive the highest quality care and attention. Responsibilities:

• Perform comprehensive initial and ongoing assessments of our residents' medical, psychological, and social needs
• Develop, implement, and evaluate resident care plans in collaboration with the interdisciplinary team
• Coordinate and document resident care services, including medication management, treatment plans, and family communications
• Provide direct patient care, including medication administration, wound care, and vital sign monitoring
• Collaborate with healthcare professionals to ensure seamless transitions and continuity of care
• Participate in quality improvement initiatives to optimize resident outcomes and minimize risk
• Maintain accurate and timely records, including MDS assessments and care plans

Requirements:

• Minimum of 1 year of experience in a skilled nursing setting
• Current RN or LPN license to practice in the state of Ohio
• MDS certification preferred, but not required
• Excellent communication, organizational, and interpersonal skills
• Ability to work effectively in a fast-paced environment with frequent interruptions
• Strong attention to detail and ability to maintain accurate records
