Position Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide direct nursing care and support daily clinical operations within our Supportive Living community.

This role includes supervising nursing assistants, ensuring regulatory compliance, and promoting a calm, resident-centered environment.


Key Responsibilities

  • Provide direct nursing care in accordance with physician orders and care plans

  • Supervise and guide nursing assistants in daily care delivery

  • Administer medications and treatments safely and accurately

  • Admit, transfer, and discharge residents as needed

  • Complete all required clinical documentation and nurse’s notes

  • Receive and transcribe physician orders

  • Investigate and report incidents/accidents per policy

  • Participate in shift reports and interdisciplinary communication

  • Maintain confidentiality of resident information (HIPAA compliance)

  • Foster a respectful, safe, and supportive care environment


Qualifications

  • Current, active Illinois LPN license (in good standing)

  • CPR certification required

  • Strong clinical judgment and ability to make independent decisions

  • Knowledge of nursing practices and Supportive Living regulations

  • Prior experience preferred, but not required

  • Strong communication and teamwork skills


Physical Requirements

  • Ability to move throughout the workday

  • Ability to lift, push, pull, or carry up to 25 pounds

  • Ability to assist in emergency situations if needed

  • Ability to meet the physical, mental, and emotional demands of the role
