About the Role

We’re seeking a compassionate Registered Nurse (RN) to join our hospice team—available full-time, part-time, or PRN. As an RN Case Manager, you’ll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families.

You’ll collaborate with an interdisciplinary team to ensure every patient is cared for—body, mind, and spirit.

About the Role We’re seeking a dedicated and experienced Clinical Manager, Extended Hours to provide leadership for critical care services during the Extended Hours schedule ( 12:00 p.m.–8:00 p.m. Monday–Friday , with Thursdays beginning at 9:30 a.m. ). This role oversees all clinical services delivered after 4:30 p.m. on weekdays, as well as 24-hour coverage on Saturdays, Sundays, and Blue Ridge Care–recognized holidays , Hospice in the Hospital, and after-hours admissions. As a Clinical Manager, you will support and guide staff, ensure continuity of care, uphold regulatory compliance, and serve as a resource and mentor. Ensure timely, accurate documentation in accordance with policy. Maintain professional relationships with referral partners, physicians, and service providers. Ensure seamless communication and continuity of care across settings. Deliver care cost‑effectively and demonstrate knowledge of hospice reimbursement and benefit structures. Manage patient and family phone calls during extended hours. Oversee medical supply processes and inventory. Participate in infection control practices and monitoring. Provide clinical and administrative leadership in the absence of other Clinical Managers. Ensure all nursing services meet regulatory, federal, and organizational standards. Serve as a liaison with staff, physicians, vendors, and contracted partners. Collaborate with payers to negotiate authorizations and services. Review, monitor, and approve payroll and manage overtime. Mentor staff on clinical issues, documentation, and problem‑solving. Communicate regularly with staff regarding updates, expectations, and organizational changes. Hospice & Palliative Care Nursing certification preferred. At least two years of hospice experience preferred. Bachelor’s degree in Nursing preferred. Strong leadership and team‑building skills. Excellent verbal and written communication abilities. Proficiency with computers and electronic medical records (EMR). Strong delegation skills—matching the right tasks to the right team members. Ability to thrive in a multidisciplinary team environment. Current CPR certification. Physical, Sensory & Cognitive Requirements Ability to stand, walk, bend, lift patients, and maintain various physical postures for extended periods. About the Role We’re seeking a dedicated Registered Nurse (RN) to join our PACE interdisciplinary team in a primarily field-based, in-home nursing role. In this position, you will provide direct clinical care to participants in their homes, assisted living facilities, and community settings, supporting older adults in maintaining safety, independence, and dignity within their preferred living environment. This role is ideal for a nurse who values autonomy, strong clinical assessment skills, and meaningful relationships with participants and families. How You’ll Make a Difference Conduct comprehensive in-home nursing assessments and ongoing evaluations of participant status. Perform skilled nursing visits including medication management, wound care, injections, phlebotomy, and chronic disease monitoring. Assess changes in condition and communicate promptly with providers and the interdisciplinary team (IDT). Develop, implement, and monitor individualized care plans in collaboration with the IDT. Provide participant and caregiver education on disease management, medications, safety, and prevention strategies. Coordinate care transitions following hospitalizations, ER visits, or facility stays. Support participants residing in ALF or other community settings as needed. Identify safety concerns in the home and collaborate with PT/OT and social work to address environmental risks. Participate in interdisciplinary team meetings and contribute to holistic care planning. Document all nursing care accurately and timely in the electronic medical record. Respond to urgent clinical needs and participate in on-call rotation as assigned. Monitor medication adherence and assist with medication setup or reconciliation. Collaborate with external providers, pharmacies, home health agencies, and specialty services. Perform other related duties as assigned. What You Bring Current Registered Nurse (RN) licensure in the Commonwealth of Virginia. Current BLS/CPR certification. Minimum of one year of experience working with the frail or elderly population. Strong clinical assessment and critical thinking skills. Experience with home health, geriatrics, hospice, or community-based care preferred. Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact. Valid driver’s license, proof of insurance, and reliable transportation (required due to field-based nature of the role). Ability to work independently while maintaining strong communication with a collaborative team.
