About the Role
We’re seeking a dedicated and experienced Clinical Manager, Extended Hours to provide leadership for critical care services during the Extended Hours schedule (12:00 p.m.–8:00 p.m. Monday–Friday, with Thursdays beginning at 9:30 a.m.). This role oversees all clinical services delivered after 4:30 p.m. on weekdays, as well as 24-hour coverage on Saturdays, Sundays, and Blue Ridge Care–recognized holidays, Hospice in the Hospital, and after-hours admissions.
As a Clinical Manager, you will support and guide staff, ensure continuity of care, uphold regulatory compliance, and serve as a resource and mentor. Your leadership directly impacts patient comfort, family experience, and the quality of our hospice services.
You will model the Blue Ridge mission, demonstrate ethical leadership, and uphold excellence across the extended-hours team.
How You’ll Make a Difference
Serve as a patient advocate with compassion and integrity.
Conduct clinical assessments for patients referred to Blue Ridge Hospice.
Train and support extended-hours staff in patient assessments and workflow.
Manage immediate clinical concerns and escalate when appropriate.
Ensure timely, accurate documentation in accordance with policy.
Maintain professional relationships with referral partners, physicians, and service providers.
Ensure seamless communication and continuity of care across settings.
Deliver care cost‑effectively and demonstrate knowledge of hospice reimbursement and benefit structures.
Manage patient and family phone calls during extended hours.
Oversee medical supply processes and inventory.
Participate in infection control practices and monitoring.
Provide clinical and administrative leadership in the absence of other Clinical Managers.
Ensure all nursing services meet regulatory, federal, and organizational standards.
Serve as a liaison with staff, physicians, vendors, and contracted partners.
Collaborate with payers to negotiate authorizations and services.
Review, monitor, and approve payroll and manage overtime.
Mentor staff on clinical issues, documentation, and problem‑solving.
Communicate regularly with staff regarding updates, expectations, and organizational changes.
Provide direct patient care as needed, including admissions, reassessments, triage, and coverage during high acuity or staff shortages.
Oversee and support Tuck‑In call processes.
Travel throughout the service area as needed.
Perform additional responsibilities as assigned.
What You’ll Bring
Current Registered Nurse (RN) license in the state of Virginia.
Hospice & Palliative Care Nursing certification preferred.
At least two years of hospice experience preferred.
Bachelor’s degree in Nursing preferred.
Strong leadership and team‑building skills.
Excellent verbal and written communication abilities.
Proficiency with computers and electronic medical records (EMR).
Strong delegation skills—matching the right tasks to the right team members.
Ability to thrive in a multidisciplinary team environment.
Current CPR certification.
Physical, Sensory & Cognitive Requirements
Ability to stand, walk, bend, lift patients, and maintain various physical postures for extended periods.
Visual acuity to assess patient conditions and read instruments.
Hearing acuity to auscultate, take instruction, respond to alarms, and communicate effectively.
Ability to concentrate with frequent interruptions, retain multiple tasks over long shifts, and process complex information.
Ability to manage stress, emotional situations, patient and family needs, and collaborate with the interdisciplinary team with professionalism.
Ability to lift up to 50 pounds.
