Position Summary

The LPN provides highquality nursing care and direct support to residents in accordance with state regulations and facility standards. This role promotes resident independence, safety, and dignity while ensuring accurate clinical documentation and communication.

Key Responsibilities

  • Provide direct resident care, including medication administration, treatments, vitals, and monitoring.

  • Document care accurately and promptly in accordance with facility and state guidelines.

  • Collaborate with the care team to support individualized care plans.

  • Respond to resident needs, changes in condition, and emergencies.

  • Communicate effectively with residents, families, physicians, and staff.

  • Maintain a safe and clean environment consistent with infection control standards.

  • Support caregivers and participate in training as needed.

  • Uphold facility values of compassion, respect, and quality care.

Qualifications

  • Current and active LPN license in the state of practice.

  • Prior experience in assisted living, longterm care, or geriatric nursing preferred (but not required).

  • Strong assessment and clinical skills.

  • Excellent communication, teamwork, and timemanagement abilities.

  • Compassionate, patient, and residentfocused demeanor.

  • Ability to work assigned shifts, including evenings, weekends, or holidays if needed.
